Dear Patient,

Welcome to Cahaba Dental Arts. We are so pleased you have chosen us for your dental care. Referrals are greatly valued by our practice. We are always excited to see new smiles come through our doors! In order to provide you with the best possible care, Dr. Jayme will complete a comprehensive oral examination upon your first visit. This includes a complete review of your medical and dental history, all necessary radiographs and intraoral images, study models, oral cancer screening, periodontal health evaluation, and an examination of your teeth. Following this exam, she will discuss her findings with you, develop a treatment plan that you are comfortable with, and then we will schedule future visits accordingly.

Please prepare for your appointment by filling out the new patient information below, as well as any dental insurance information that you may have. Also, please read our financial policy. We are happy to answer any questions you may have about finances. Once scheduled, we reserve your time slot exclusively for you, so we ask that you give a 48-hour notice if you need to change your appointment time.
